Care that starts close to home
Proxima Home Health Care was founded with a simple belief: people deserve the opportunity to heal, regain confidence, and live with dignity in the comfort of home. Familiar surroundings, trusted routines, and the presence of family can make an important difference during recovery and in the long-term management of health needs.
Our company brings together two complementary perspectives. One founder has a Ph.D. and a strong background in scientific research, critical analysis, and evidence-informed decision-making. The other is a Registered Nurse who previously helped found and build a successful home health organization in Pennsylvania, bringing firsthand experience in clinical leadership, patient care, caregiver support, regulatory expectations, and the day-to-day realities of operating a dependable home health agency.
Together, that combination of science, nursing, and operational experience shapes the way Proxima approaches care. We believe quality home health requires more than completing a visit. It requires careful assessment, reliable communication, thoughtful caregiver matching, respect for the plan of care, and a willingness to listen when patients and families explain what they need to feel safe and supported.
Our name — Proxima, meaning "nearest" — reflects our commitment to being a close, responsive care partner for patients, families, employees, physicians, and referral sources. Every care relationship is different, but our guiding question remains the same: what is best for the patient? That question informs how we coordinate services, communicate changes, support our staff, and continually improve the experience of receiving care at home.
Based in Belle Vernon, Pennsylvania, Proxima serves communities across the Pittsburgh, Erie, and Harrisburg regions. We are committed to applicable licensing, privacy, safety, and quality requirements while building a culture grounded in our HEARTS values: Home-Centered care, Excellence, Accountability, Respect and Compassion, Teamwork with Families, and Strength Through Growth. Most importantly, we strive to treat every patient and family with the same attention, honesty, and dignity we would want for our own loved ones.
